As part of this commitment, we ask that you provide us with information about your past medical history so we have the knowledge we need to provide the most appropriate medical treatment, as well as information that can be readily accessed in case of an emergency.

Prior to your first consultation your will need to complete a registration form. To save time on your arrival please complete the form online or to download and complete a paper copy. Alternatively please arrive 10 minutes before your appointment to fill in a form at the clinic.

Patients Request To Reprint For Any Forms, Medical/Carer Certificates, Scripts, Referrals Or Requests – A Private Fee(Non-Medicare Rebate Able) $20.00 Will Be Charged.

All dressings Will Be Charged As Per Cost Of Dressing Items.

ARE STILL AVAILABLE FOR PATIENTS WHO HAVE HAD A FACE-TO-FACE APPOINTMENT WITH A DOCTOR AT THE PRACTICE IN THE LAST 12 MONTHS OR WHO HAVE TESTED COVID POSITIVE.
